PURPOSE

This form is for students or their parents/guardians to report a hate or bias motivated incident and/or a bullying incident involving or impacting HCDSB students, either as victims, witnesses, or alleged perpetrators. (Procedure VI 102 Responding to Hate or Bias Motivated Incidents Involving or Impacting Students; ). 

A hate or bias motivated incident is a non-criminal action (does not involve violence, threats or property damage) committed against a person or property that is motivated in whole or in part by hate and or bias based on membership in a Code Protected ground such as race, ancestry, place of origin, colour, ethnic origin, citizenship, creed, age, disability, sexual orientation, sex, gender identity, gender expression, family status, marital status or socio-economic status. The conduct can be verbal or non-verbal and may include jokes, innuendos, pictures, teasing, ridiculing, etc.

A bullying incident is a 鈥渂ehaviour that can be repeated or occur one time and can be carried out by an individual or group of individual鈥. (Ontario Ministry of Education, PPM 144).

By completing the form, you acknowledge that the information will be shared with the School Principal or the Family of Schools Superintendent and that you will be contacted for more information and next steps. If you choose not to report anonymously and you have provided your contact information, the Principal or Family of Schools Superintendent will respond to you within 3 school days of receiving the report. If this report is submitted during a period when schools are closed (holidays, March Break, summer months, etc.) then you will receive a response once school resumes.
